Social Security Number
Provide your Social Security Number. If you have a SSN issued by DACA, do not include it on your application. If you are not a U.S. citizen, you will also need your Alien Registration Number or visa information.
Previous Education Information
Names and dates (month and year) of all high schools and colleges attended.
Email Address
Confirmation and follow-up information of your application will be sent via email. If you do not have a valid email, please create one prior to applying.
Within 24 hours of applying you will receive a Welcome to the 黑料专区 email with your student ID number and mySiskiyous student portal login information.
